Grant Criteria

  • In order to apply for a grant from the ProCure Cancer Foundation (“Foundation”), the applicant must submit the following materials for their application to be considered complete:
    1. ProCure Cancer Foundation Application(form is available for download here); and 
    2. A copy of the applicant’s household tax return filed with the IRS for the previous year; and
    3. Demonstration of the applicant’s acceptance for treatment at one of the existing proton radiation therapy facilities in the U.S. to be completed by an authorized employee of the proton radiation therapy center (form is available for download here); and
    4. Copies of the applicant’s statements for all checking, savings, and brokerage accounts for the past two months.

    The Foundation will not review applications that do not contain all of the materials listed above.

    Please save receipts for all items you are requesting assistance for.

  • The Foundation evaluates applications on a monthly basis. In order to ensure that your information is reviewed in the quickest manner possible, please submit your completed materials by the first of the month. Materials received after that date will not be reviewed until the following month. If you are awarded a grant, you will receive a phone call from the Foundation informing you of its decision.
  • Funds disbursed by the Foundation may only be used for non-medical expenses associated with receiving proton radiation therapy that are not covered by medical insurance. The funds cannot be used for copayments, deductibles or other medically related expenses. Permitted uses of funds include:
    1. Transportation to and from the proton radiation therapy center (may include reasonable airfare, taxi, or car rental charges)
    2. Customary accommodation costs
    3. Meals and incidental expenses (daily meals and incidental expenses incurred by the applicant while undergoing proton radiation therapy should conform to the per diem amounts set by the U.S. General Services Administration.
